We also ask that you bring a list of any medications you're currently taking. If you have dental insurance, please give us that information when you schedule your appointment. After you are checking in and have submitted your new patient forms, we will take you back and make any necessary records, including X-rays, photographs, and scans of your mouth. You will then meet Dr. Hackbarth, who will examine you and consult with you about your smile and oral health. Then we will develop a personalized treatment plan for you.
