We want your first visit to be as easy as possible. If you need special accommodations, please let us know at the time you set up your appointment. As with any medical office, there are patient forms you need to fill out. If you would like to start on your paperwork before you arrive, please see the downloadable forms below. We also ask that you bring a list of any medications you're currently taking. If you have dental insurance, please give us that information when you schedule your appointment. After you are checking in and have submitted your new patient forms, we will take you back and make any necessary records, including X-rays, photographs, and scans of your mouth. You will then meet Dr. Hackbarth, who will examine you and consult with you about your smile and oral health. Then we will develop a personalized treatment plan for you.
